IMPORTANT: 

If you are sending a form to be completed, please make sure the “Patient Information” section is completed and signed prior to sending.  Do not send a blank form.  We are unable to complete your request without this information/authorization.

If you need to send a form or document to have completed by your physician, please send in PDF format.  Picture formats such as JPEG or PNG often have shadows and are too large to upload into our system.  If you do not have a scanner available, please follow the instructions below to create a PDF file:
